Cash versus insurance
Some Montana practices that advertise men’s HRT are cash-pay. In the verified 2026-08-31 directory, Alliance Health in Billings lists cash and no insurance, and Whitefish Vitality lists cash-only TRT. Other offices may bill insurance for an evaluation or for labs and still expect you to pay cash for medication or a membership. Ask which parts they submit to insurance and which they do not. We do not have a verified coverage list for every clinic, so we omit it.
Insurance sometimes covers a medically indicated workup — a visit and clinician-ordered labs — when a clinician documents a reason to test. That is not the same as covering ongoing testosterone from a cash men’s clinic. Your plan’s medical policy, not a marketing page, decides.
What a quote should include
Memberships and “programs” often bundle some visits and leave other costs out. Ask for a plain list that separates:
- The first visit or evaluation
- Laboratory draws and who bills them (clinic, hospital, or independent lab)
- Medication, if anyone is prescribed it — and the form
- Follow-up visits and repeat labs
- What happens if you are not a candidate after labs

Labs are often a separate bill
In-town clinics may draw blood on site. Telehealth and many primary-care paths send you to a hospital or independent draw station. That bill can be cash or insurance depending on the lab and the order. Ask who ordered the labs and who you pay. A “membership” that looks cheap can still leave you with a lab invoice.
Travel is part of the cost in this state
A weekday appointment in Billings, Bozeman, or Missoula is a lost workday if you live two hours out. Licensed telehealth can cut travel. It does not cut the need for a clinician or a lab draw, and it is not automatically cheaper — ask the same itemized questions.
Questions that get you a real number
Take the cost questions on our visit list. If a clinic will not say what is not included, you are in a sales process. If they can tell you what they would charge if labs say you are not a candidate, that is useful information too.
